I have attached the Schedule Selection form below. As you know we are offering a total of 14 classes in 5 time slots. You asked for variety and I believe this year you are going to get it. Because of the multiple classes we will have to have a schedule completed by each attendee to insure they get into the classes they want to attend.
As a note sessions held in Classroom 2 and 3 are limited seating so if you are wanting those classes you need to complete this form and return it quickly. We will be filling the classes in the order in which these forms are returned.
Please complete one schedule per attendee. If you have several people wishing to attend the same classes you can use one form but please make sure their names are clearly noted. Attendees without a completed schedule will be placed in open classes. If you register for a class that is full you will be placed in the next available class unless you note a secondary preference.
HOW TO COMPLETE THE FORM. Complete the form on your computer then save it to your computer. Next send an email to email@example.com and attach the saved file. In the body of the email please note any specific second choices you may have for classes. If you prefer you can print out the form and fax it to Peggy at 850.434.0034. I do not suggest mailing the forms in regular mail as space is limited in some of the classes and will be filled in the order in which the forms come back.
